Section 1

The Town MD Privacy Policy

Our Privacy Policy explains how we collect, use, protect, and share personal and health information when you use our website, services, clinic communications, and related patient resources.

Section 2

Consent and Your Choices

How Does Our Privacy Policy Address Consent?

When you provide personal information to schedule an appointment, verify insurance coverage, or complete a transaction, we interpret that action as agreement to collect and use the information for the specified purpose. If we request information for a secondary purpose, such as health newsletter updates, we ask for your expressed consent or give you an immediate opportunity to decline that secondary use.

When you use our online portal, we also automatically receive your computer’s Internet Protocol, or IP, address. This technical information helps identify details about the browser and operating system used to access our online services. The information is collected as part of your interaction with the portal rather than through a separate form or manual entry.

If you give us permission, we may send you emails or texts about meetings, changes to services, and clinic news. We treat these communications separately from information collected for a specific appointment or transaction. People who can be identified do not have their information sold, shared, or traded with other companies or affiliates for marketing or advertising reasons.

Section 3

Consent and Your Choices

How Does Our Privacy Policy Address Consent?

When you provide personal information to schedule an appointment, verify insurance coverage, or complete a transaction, we interpret that action as agreement to collect and use the information for the specified purpose. If we request information for a secondary purpose, such as health newsletter updates, we ask for your expressed consent or give you an immediate opportunity to decline that secondary use.

How Can I Withdraw Consent?

If you change your mind after opting in, you can withdraw your permission for us to contact you or to continue collecting, using, or disclosing your information. You can reach our Privacy Compliance Officer at hrptrehab@gmail.com or write to The Town MD at 119 E Passaic Street, Maywood, NJ 07607.

Section 4

Disclosure and Payment Handling

We may disclose your personal information when required by law or when you violate our Terms of Service. These are the circumstances identified in our policy for legal disclosure. They do not create a separate marketing or promotional permission to share information that you provide to us for healthcare, scheduling, payment, or other stated purposes.

For payment security, if you select a direct payment gateway for copayments, coinsurance, or self-pay services, your payment information is encrypted in accordance with the Payment Card Industry Data Security Standard, or PCI-DSS. Transaction data is stored only for the period necessary to complete the purchase transaction and is then securely purged. The direct payment gateways referenced in our policy follow PCI-DSS standards managed by the PCI Security Standards Council.

Section 5

Third-Party Services and Links

The third-party providers we work with may collect, use, or disclose information only to the extent required to perform the clinical, scheduling, or technical services they provide to us. Examples identified in our policy include Electronic Medical Record platforms, payment gateways, and insurance verification portals. Certain providers maintain their own privacy policies governing information required for purchase or scheduling transactions.

Links on our website, including map links and external health portals, may direct you away from our site to websites we do not control. We are not responsible for the privacy practices of those outside websites. We encourage you to review the privacy statement of any external website before deciding whether to provide personal or other sensitive information.

Section 6

Data Security and Encryption

To protect personal and health information, we take reasonable clinical precautions and follow industry best practices designed to reduce inappropriate loss, misuse, access, disclosure, alteration, or destruction. These measures apply to information covered by this policy and are part of our approach to patient privacy throughout our online interactions and related information-handling practices.

Information transmitted through our online appointment forms and portal is encrypted using Secure Socket Layer, or SSL, technology and stored with AES-256 encryption. No form of internet transmission or computer storage is completely safe. Our policy also states that we follow applicable healthcare data standards and maintain technical and administrative safeguards for the information we handle.

Section 7

Age of Consent

You confirm that you are at least of legal adulthood in your own state or province by using our website. You may also use the site as an adult who has provided consent for minor dependents to use the site or receive healthcare evaluations under your guardianship, as described in our policy.

Section 8

Changes to Our Policy

We may modify this Privacy Policy at any time, so we encourage visitors to review it periodically. Changes and clarifications become effective soon after they are on the website. If we make material changes, we update the policy so visitors can remain informed about the information we collect, how we use it, and the circumstances under which it may be disclosed.

Section 9

Information we collect through SMS:

  • We collect your mobile telephone number when you provide it for appointment coordination or when you send us a text message.
  • We may collect the content of appointment confirmation, rescheduling, or clinic inquiry messages that you send or receive.
